How to Make An Appointment

Appointments can be made by calling our office at (203) 869-1285. We schedule appointments Monday through Friday, 8 AM through 5 PM. Please be prepared to provide your accurate address, phone number, date or birth, type of insurance, reason for appointment, and referring physician, if required by your insurance.

If you are having an emergency or would like to speak to the physician on call after 5 PM, please call our office and you will be helped by our medical answering service. We have a physician on call 24 hours a day and 7 days a week for your convenience.

Patient Registration Forms

To save you time on the day of your appointment, you may print the following forms directly from our website, complete them at your convenience prior to your appointment, and bring the completed forms with you on the day of your appointment. Please be assured that NO information is being transmitted over the Internet; you are merely printing a blank form.

  • Notice of Privacy Practices (as required by the privacy regulations created as a result of the Health Insurance Portability and Accountability Act of 1996)
Test Results

Each test result is thoroughly examined and reviewed by one of our physicians to ensure complete and accurate diagnosis. Some tests can be processed as quickly as 24 hours, but others can take up to a week. After your results have been read and interpreted, you will be informed either by phone or office consultation If you have any questions regarding a diagnosis or test result, please contact our office.

Refills

Please call our office during normal office hours to request a refill on your prescription. Each request is carefully reviewed by our medical staff to ensure your safety. Office visits are sometimes required in order to refill a medication. In certain cases, we will refill a prescription for 30 days.

Billing and Insurance

Payment is expected at the completion of services. We accept cash, personal checks, and all major credit cards except American Express. If there are extenuating circumstances, please contact our billing department BEFORE services are rendered to discuss payment options.

Referrals

Many insurance carriers require that you obtain a referral prior to your office visit. In order to make your visit to us as smooth as possible, please check with your provider about their policies before your appointment. As we are not a primary care practice, we do not give out referrals.
